BrainWare Cognitive Rating Scale (Ages 9-12)

This is the BrainWare Cognitive Rating Scale you should complete BEFORE your child starts playing BrainWare SAFARI.

The purpose of this rating scale is to enable a parent or teacher to evaluate a child on behaviors that typically relate to cognitive development in key areas. It is intended to be used before your child begins using BrainWare SAFARI.  You should then complete the AFTER version of the Rating Scale after about 4 months of use, following the recommended protocol of 3 to 5 times a week, 30-60 minutes per session, for 12 -14 weeks (a minimum of 30 hours).

Please read each of the statements and rate the child according to the following scale. After you have rated the child on all statements, we will calculate a score for this child in each area.
