BrainWare Cognitive Rating Scale Adult

This is the BrainWare Cognitive Rating Scale you should complete BEFORE the individual starts playing BrainWare SAFARI.

The purpose of this survey is to evaluate an individual on behaviors that typically relate to cognitive development in key areas. It is intended to be completed before the individual begins using BrainWare, and then again after 3 to 4 months of use. The Rating Scale can be completed by a teacher or counselor or by the individual him/herself. The same person should complete the rating scale after 3 to 4 months as at the beginning.
